Saw this article of 12 Embarrassing IT mishaps of the last 25 years or so. I can think of a few others thats not included, like the F16 that flips upside down whenever it crosses the equator, and so many Microsoft demonstrations that went horibly wrong (Xbox not working, Win98 chrashing, Voice recognision that turns violent, etc).
